1095-C Tax Form
Northwell Health is required to annually report to you and to the IRS on the medical coverage offered to eligible employees and their dependents. Full-time employees (including unions*) who are eligible to participate in the Northwell Health benefit plan will receive this form. A full-time employee defined by the IRS is an employee who works 30 hours or more per week on average annually. If you do not meet this hour requirement (including per-diem and part-time employees not benefit eligible), you will not receive a form.
This reporting was be sent to your home on IRS Form 1095-C by March 4, 2019.
You do not need the form when filing your taxes, however, you should keep the form with your tax records in case you are audited by the IRS. You would need the form to prove to the IRS that you had medical coverage for the entire year. If were not covered for the entire year, you may subjected to a penalty from the IRS.
* If you are covered by a union, you will receive the 1095-C Form stating that you were not offered coverage by Northwell Health (your employer) – those union employees may have received a 1095-B Form from their union, stating the coverage offered to them.
