A Health Risk Assessment (HRA), is a 100% confidential and comprehensive questionnaire that assesses key areas of your health and lifestyle, such as nutrition, sleep, stress, and heart health. Your HRA results provide you with information that connects your lifestyle habits to risk factors for chronic conditions.

So why take Northwell’s HRA? The top three reasons are:

Know where you stand: You’ll gain a better understanding of your own health status (a wellness score and full report) that you can discuss with your health care team.

Personalized reporting: Health status highlights, tailored guidance, and recommended tools for areas of improvement, and the ability for real-time updates any time you want to record updates.

Take it to the next level: Having all this information in one place, in an easy-to-understand format, could be just the motivation you need to take your health and lifestyle to the next level.

Bonus! If you are enrolled in a Northwell employee medical plan*, taking the HRA is the gateway wellness action in the Wellness Credit Program. This means it’s a required action if you want to work toward earning the maximum amount in 2020 paycheck credits for completion of four healthy actions by Oct. 31, 2019.

Ready to take the HRA? Go to Northwell.edu/myWellness and click on the “Programs” menu item. HEALTH RISK ASSESSMENT will be at the top of the Programs page, click START NOW. The HRA should take no more than ten minutes to complete.

Once you’ve taken the HRA, myWellness/Virgin Pulse serves up customized content to help you focus on areas that need improvement. As always, myWellness is a great way to track your progress, earn points, and connect with team members through a variety of challenges.
